Contraindications
- Anuria, renal decompensation, hypersensitivity to thiazide and related diuretics or sulphonamides, hepatic coma, hyponatreamia, refractory hypokaleamia, hyperuricaemia, addison's disease.
Precautions
- May aggravate diabetes, gout & systematic lupus erythematous.
- May cause hypokaleamia.
- Pregnancy and lactation, elderly, renal/hepatic impairment, withdraw several days before commencing ACE inhibitor treatment.
Point of care
Dosing
Adult
Oedema: 5-10mg OD or on alternate days, maintenance: 2.5-10mg 1-3 times weekly. Hypertension: 2.5mg OD (Higher doses may not be required).
Paediatric
See label paediatric section if present; otherwise use paediatric formulary — do not extrapolate adult doses.
Renal
Review renal impairment dosing; many agents need CrCl/eGFR adjustment.
- CrCl 0–120: Confirm renal dosing in product SmPC / primary label.
Hepatic
Review hepatic impairment dosing; monitor LFTs if agent is hepatically cleared or hepatotoxic.

Adverse effects
- GI disturbances, hypokalaemia, manifestations of latent diabetes, sensitivity reactions, blood dyscrasias, postural hypotension, reversible impotence, gout, pancreatitis, blurred vision, muscle spasm, hypomagnesia, natraemia and chloraemic alkalosis, and hypercalceamia.

Indications
- Hypertension, oedema, nephrogenic diabetes insipidus, osteoporosis in postmenopausal women, calcium nephrolithiasis.
- Clinical selection for Bendroflumethiazide should follow culture results where relevant, Kenya STG/EML recommendations, and the current product SmPC.

Pharmacology
Mode of action
Inhibits reabsorption of sodium and chloride ions in the cortical thick ascending limb of the loop of henle and early distal tubules.

Inhibits reabsorption of sodium and chloride ions in the cortical thick ascending limb of the loop of henle and early distal tubules. It has direct antihypertensive effect requiring several days to be manifested.
